Application for Entrance Test
(Incoming High School Freshmen)
1. Secure an application form from the Registrar’s Office. This form is free of charge and can be photocopied.
2. Fill-out the form completely and attach:
a. A clear photocopy of the applicant’s Grade 5 and latest Grade 6 report cards
b. Two (2) pieces of 2x2 colored I.D. pictures
3. Pay the testing fee at the Cashier’s Office
4. Submit the form with the above requirements and receipt to the Registrar’s Office. Thereafter a test permit indicating the schedule of entrance test will be issued to the applicant.
5. Applicants should report on their scheduled examination date at least 10 minutes earlier than their scheduled examination time. They should bring the test permit, pencil and eraser.
6. Applicants should follow-up on the application 2 weeks after taking the exam. A list of qualifiers and other possible candidates will be posted on the bulletin boards.
7. Qualified freshmen applicants will be advised to report for pre-enrollment.
8. Applicants with pending or wait-listed applications or possible candidates for enrolment will report for an interview with the Principal/Assistant Principal. Only after satisfactorily passing the interview can they be advised to pre-enroll.
Application for Entrance Test
(2nd & 3rd Year Transferees)
1. Transfer applicants for 2nd year level must present to the Registrar a clear photocopy of Grade 6 and latest First Year Report Cards, while those applying for 3rd year level need to submit a clear photocopy of First year and latest Second year Report Cards.
2. Transfer applicants will undergo a preliminary interview with the Principal before they can secure an application form.
3. Only after satisfactorily passing the initial interview can the applicants proceed in their application.
a. Secure an application form from the Registrar’s Office
b. Fill-out the form completely and attach:
i. A clear photocopy of the required Report Cards
ii. Original copy of Good Moral Certificate
iii. 2 pieces of 2x2 colored I.D. pictures
4. Pay the testing fee at the Cashier’s office
5. Submit the form with the above requirements and receipt to the Registrar’s Office and wait for the test permit.
6. Report to the Guidance Office for testing on the date specified on the test permit issued.
7. Result of test will be released two weeks after the date of exam. Qualified transfer-applicants will then be informed of the date of pre-enrollment.
